All confidential information must be redacted from the original petition and submitted on green paper per Admin Rule 9. You will be given the date of the name change hearing when you file the case. It will be your responsibility to have the Notice published according to the Indiana state law (IC 34-28-3). At the hearing you will need to have the original order and four (4) copies for the Judge’s signature.PLEASE NOTE: The Clerk's office web site does have a link to the State web site that contains forms that may be used for Change of Name.
