Requirements for Filing a Name Change Petition

 

PLEASE NOTE: The Clerk's office does have the appearance and the notice form needed as outlined below (see the civil section under 'Forms' on this Web site's navigation bar to download a copy). However, the Clerk's office does not have a 'form' available for a petition for change of name. Any law library should be able to provide books that would enable a person to research the law to know what language the petition for change of name should contain and the legal requirements for publication. Should legal assistance be required, please seek the advice of an attorney.

Petition: Original and two (2) copies. (Typewritten on 8 ½ X 11 inch white paper)

Notice of Petition for Change of Name by Individual: Original and two (2) copies

Appearance Form: Original and one (1) copy.

Filing Fee: $136 (cash or money order) paid at the time of filing.

Publication Fee: Contact the paper you are going to use.

You will be given the date of the name change hearing when you file the case. It will be your responsibility to have the notice published according to the Indiana Statute. This should be taken care of as soon as possible.

At the hearing you will need to have the following for the Judge's signature:

Order: Original and four (4) copies
