Birth Certificates upon Paternity Judgements

The Clerk's Office is required by law to send to the Indiana State Board of Health a Birth Change Order within thirty (30) days of the Paternity Order.  The required information for the filing of a paternity action enables the case to be processed promptly and assists the Clerk's Office in preparing the necessary form so a new Birth Certificate can be issued.

To obtain a new Birth Certificate, present the child's original birth certificate to the Board of Health, along with the Court Order showing the name change.  A new Birth Certificate will then be provided (at no charge).  If the original Birth Certificate is not available, the Board of Health charges $6 per certificate.  The Board of Health will also laminate the document for an additional $1 fee.
