Marion County Child Support Forms

All Marion County Child Support Forms can be returned by US mail or in person:

Electronic Funds Transfer (EFT) Packet

Contains information and form to have Non- Custodial Parent’s child support payment directly deducted from savings or checking account.  A clear copy of a state issued picture ID must be presented w/ form.  A voided check or bank deposit slip must be included w/ form.

Electronic Funds Transfer (EFT) Change Request For Non-Custodial parent to change amount, frequency, debit date, bank information, or to close  an existing EFT account.  A clear copy of a state issued picture ID must be presented w/ form.  All bank information changes must be requested w/ a voided check or bank deposit slip.
Name or Address Change Form For Custodial or Non-Custodial Parents; this form requests address and/or name changes.  A clear copy of a state issued picture ID must be presented w/ form.
Contact Information Sheet Lists resources related to child support, legal aide, and pro se forms.
Child Support Account Information Sheet For Attorney and Pro Se use.  This form is required to set up an account w/ the Marion County Clerk’s office for a new child support case.  A copy of the court order establishing child support must be presented w/ form.
Child Support Fact Sheet Useful information on child support payments, payment methods, and docket fees.


State of Indiana Child Support Forms

Forms listed below are found on the State of Indiana’s Child Support Payment Processing Web site (http://insccu.com), and are available below for your convenience.  All payments made by check, money order or cashier’s check, including employer payments, should be mailed directly to:

Employer Form To be sent by employer for payments withheld by employer for child support. 
Non-Custodial Parent Payment Coupons  Coupons to be sent in with checks, money orders, or cashier’s checks.  Contains all information needed to process non-custodial parent’s child support payment.
Custodial Parent Direct Deposit Form  Child Support Payments to be directly deposited to Custodial Parent’s checking or savings account.  
Eppi Card(English)
Eppi Card (Español) 		 Information about having your child support payment directly deposited to a state issued VISA debit card.
