Marion County Child Support Forms

All Marion County Child Support Forms can be returned by US mail or in person:

Name or Address Change Form

​For Custodial or Non-Custodial Parents; this form requests address and/or name changes. A clear copy of a state issued ID must be presented with form.
Contact Information Sheet ​Lists resources related to child support, legal aide, and pro se forms.
Child Support Account Information Sheet ​For Attorney and Pro Se use. This form is required to set up an account with the Marion County Clerk's Office for a new child support case. A copy of the court order establishing child support must be presented with form.
Child Support Info Sheet ​Useful information on child support payments, payment methods, and docket fees.
Arrearage Calculation Worksheet ​For non-custodial parents to help calculate arrearage amount.
 

 

 

State of Indiana Child Support Forms

Forms listed below are found on the State of Indiana’s Child Support Payment Processing Web site (http://insccu.com), and are available below for your convenience.  All payments made by check, money order or cashier’s check, including employer payments, should be mailed directly to:

​Employer Payment Remittance Form ​To be sent by employer for payments withheld by employer for child support.
Employer Support Payment System ​To be used by employer to transfer child support funds electronically.
​Non-Custodial Parent Payment Coupons ​Coupons to be sent in with checks, money orders, or cashier's checks. Contains all information needed to process non-custodial parent's child support payment.
